We would like to inform the DAO that on April 19, 2026, one of our Lido validator infrastructure servers in the São Paulo region experienced a critical hardware failure affecting the operating system disk. The incident caused a temporary disruption in validator operations and ultimately resulted in approximately 1.224 ETH in missed rewards. As part of our commitment to operational accountability, SenseiNode will fully compensate the DAO for the associated loss.
Following automated alerts indicating filesystem I/O errors, our engineering team immediately initiated incident response procedures and coordinated with the datacenter provider to diagnose and remediate the issue. The root cause was confirmed to be a terminal failure of the primary OS storage device due to physical degradation/block corruption, to the point where the disk was no longer recognized by the system firmware. Throughout the recovery process, special care was taken to preserve all blockchain data volumes and validator state storage.
Importantly, our infrastructure architecture isolates the operating system from validator and blockchain data volumes, which ensured there was no loss of validator keys, chain data, or security compromise during the event. After the failed disk was replaced and the system reinstalled, validator services were successfully restored the same day and all integrity checks passed. We have since reviewed and strengthened our hardware lifecycle monitoring and recovery procedures to further reduce the risk and recovery time of similar OS-level hardware incidents in the future.
This incident, while unforeseen, underscores the transparency and accountability that have defined our trajectory.
